The FDA found that the Chinese heparin was "contaminated" with Chondroiten, a cheaper substance that tests similarly to Heparin.

This is not an accident...like other medication contamination (and the pet food contamination) it took someone who knew how quality testing was done, and then the cheaper ingredient that could pass the tests was substituted to make a profit.
