then there is the question of privacy in medical records...link2...
we had our (MS DOS) records get infected ten years back, and it was a mess. Of course, this was only the summary of patient problems and what medicines they were taking, and we still had the large paper charts, but still, it was a mess.
I suspect most offices will end up using a "cheat sheet" for each patient, so we don't get too lost in the trivia that the government now asks us to document.
And, of course, a lot of the really private things won't get into the medical records....
